In past years, the Inter-Lakes School District has provided a 1095-C form to employees with district medical coverage.  This form showed that the District offered affordable, minimum essential coverage for the previous calendar year.

As of January 1, 2025 (for calendar year 2024) the District is only required to provide 1095-C forms if requested by the employee.  Please note that this IRS form is not required to be filed with individual tax returns.  If you would like to receive a 1095-C form, please contact Rebecca Stopyra by email at rebecca.stopyra@interlakes.org.  All requests will be processed within 30 days.
